Evaluation the benefit of kalazar detect rapid test for detection of visceral leishmaniasis in Al-Diwaniah province/Iraq

Abstract

The aim of study is to determine the benefit of In Bios kala-azar detect rapid test in comparative with IFA test in cases with clinical suspicion of visceral leishmaniasis. Eighty  four patients with clinical diagnosis of  were studied in children and maternity teaching hospital in Al-Diwaniah/Iraq  for children  and all specimens were checked by using the Kala-azar  detect  rapid test and rechecked after that by using immunofluorescence antibody test, after  that  the  patients  sera  were  sent  for immune-flourescent antibody tests test to confirm or to exclude the diagnosis of the kala-azar. In a current study of eight four (84) patients were clinically suspected as kala-azar (visceral leishmaniasis) using the main two common laboratory investigation (InBios kala-azar detect rapid test and IFA test). The diagnosis of visceral leishmaniasis depend on clinical suspicion of the common signs and symptoms of the disease in areas highly endemic with kala-azar; hepatomegaly in 40/84 patients (7.6%) and splenomegaly founded in 81/84 (96.4%). Fever and pallor were detected in almost all patients through the first clinical attack of visceral leishmaniasis 84/84 (100%). Kala-azar detect rapid test truly  positive only in about 72/84  with  sensitivity  reach  (85.71%) the  in  compares  to  other test;  direct immuno-flourecenc antibody (IFA) test  in which the sensitivity of the test reach to (98.8%); and the InBios kala-azar detect rapid test appear positive in  only one case from control  group thirty (30), diagnosed clinically as a typhoid fever  in which the specificity of the test were (96.9 %). In conclusion, the kala-azar detect rapid test is a sensitive, not invasive, rapid, easy to give result within minutes and low cost diagnostic method to be used for the diagnosis of kala-azar but should not use as  the  sole  criterion  for the  diagnosis   of the visceral leishmaniasis  unless the  signs  and  symptoms  present.

Abstract

Tuberculosis (TB) stands as a major public health problem worldwide. The rapid diagnosis of individuals with active TB and detection of individuals with latent TB and rapid introduction of effective treatment to prevent them from spreading of disease. The aim of study was to evaluation the IGRA in the diagnosis of TB in comparison to other tests (conventional and molecular tests) during the period from October/2015 to June/2016 in advisory clinic for respiratory diseases and thoracic in Al-Samawa city in Al- Muthanna province. Sixty sputum and blood specimens were taken in this study was conducted with randomly selected people for evaluation of immunological detection of active and non-active tuberculosis compare with control individuals by using T-Spot. TB test, compare with tuberculin skin test (TST), conventional and molecular tests. The diagnostic sensitivity and specificity that estimated for T-Spot.TB test compare to TST were 51% and 47% respectively, and ORA was 50 %. This study concludes capability of T-Spot.T B test in diagnosis of active, under- treatment and latent PTB cases with more benefit to diagnose of extra pulmonary cases. Also, the study confirmed the failure of TST test in evaluation of PTB positive and negative incidences in respect of vaccinated or non- vaccinated status.

Abstract

Osteoarthritis is a chronic disease characterized by degradation joint that may cause cartilage loss and morphological deterioration to another joint tissue, more delicate biochemical changes occur in the early stages of Osteoarthritis progress. healthy cartilage content water that make balanced by compressive force driving water out while hydrostatic and osmotic pressure drawing water in. Collagen fiber make the pressure force and cartilage proteoglycan create osmotic pressure which draw water in, Without the protective effect of the proteoglycan the fiber and collagen of the cartilage can become susceptible to degradation and degeneration. Hyaluronic acid is major component of synovial fluid and articular cartilage, and is responsible for viscosity and elasticity of synovial fluid. The quantity and molecular weight of Hyaluronic acid in the synovial flu-id is decrease in the osteoarthritis patient. The purpose of injecting intra-articular HA is to replace HA so that the natural viscosity of synovial fluid is maintained, so relieve pain and improve function of knee Osteoarthritis. HA injections have been used in treating knee osteoarthritis for over two decades in most developed countries, but in Iraq begin used late and slow due to lack of patient education about the injection, study diagnosed two group of knee Osteoarthritis for both gender and observed benefit and effectiveness to pre-and post of HA injection, the results were various significant during one month under study.

Abstract

Cancer is the DNA replications mistakes, where it’s enhance by many chemical factors, environmental factors, radiation, in addition to, the viral and immunological diseases. It has been diagnosed many types of cancers in human being, called by the organs and classified to breast cancer, lung cancer, prostate cancer, and colon cancer. etc. Oncology is the science of genetic materials disease after birth in humans, concern with all metabolic steps guide to abnormal DNA multiplications within somatic cells. To stop these abnormal multiplications, the researchers tend to find genetic drugs within telomerase line of research; chemical drugs used to kill the cancer cells at early stages of disease; immunological drugs that used in immune system stimulation materials. Organ transplantation and cell regenerations line of research open the door for new idea in cancer therapy, its stem cell therapy. It has been well known procedure since the 90s of last century, but the researcher has developed in Iraq since 2007 by Al-Aloachi and colloquies. It has been created many types of bone, blood, placenta stem cells, isolated, cultivated, and used as drugs for regeneration in tissue cultures. Others as Kasim in Al-Nahrain used the line in Cancer therapy.

ECG abnormalities in first visit of diabetic patients in Al Sammawah Diabetic center: cohort study

Abstract

Diabetes Mellitus is a very common progressive endocrine disease in Iraq (and our society) as part of whole world, and diabetic patients are more likely to have cardiovascular diseases than patients without diabetes. ECG is the most widely used simple noninvasive test for evaluation of cardiovascular risk, so our study was focused to assess the relation between the frequency of ECG changes with the patients glycemic control and their age group. A 600 patients with newly diagnosed diabetes Mellitus were identified from diabetic center in Al Sammawah teaching hospital. All are evaluated for variables; age, gender, glycemic control (HbA1c level) and ECG abnormalities. Then they are divided in two groups those with and others without ECG changes during evaluation of their above variables. The selected patients are 294 female and 309 male with most age distribution affected are 50-59 years 189(32%) followed by 40- 49 years,130(22%)then 60-69 age group119 (20%), 30-39 years 68(11%), >70 years 52 (9%) and <30 years about 37(6%). 327 patients with normal ECG and 273 patients with abnormal ECG, female was predominant by 55%, and ECG abnormality was distributed among low voltage in 110 patients (18 %), bradycardia in 54 patients (9%), with variable percent for the other ECG abnormalities. Diabetic control for most of the patients was poor according to HbA1c level which measured in the first visit and 77% of patients with HbA1c above 8. In conclusion; the age group distribution in Samawah city is comparable to the other population, but with very bad diabetic control which require more effective educational and advance therapeutic approaches to control this propagating challenge. ECG changes seen were predominantly low voltage, then bradycardia which may reflect other association which require more future research..
